Atlantis serves people who are very severely disabled; many of them are multiply-disabled and previously institutionalized. For this reason, a unique range of services has been developed.

HOUSING SERVICES
As needed, charge to client as appropriate.

1. Assistance with locating accessible, affordable apartments.
2. Referral for access renovation.
3. When available assistance with appliances, furniture, home repair, and maintenance.
4. Landlord-tenant relations, assist clients.

DAILY LIVING AND HOME MANAGEMENT
As needed, funding provided by contract and benefits.

1. Daily assistance, training in cooking, house keeping, laundry, other in-home activities.
2. Grocery shopping, list making and meal - planning, nutrition.
3. Budgeting, financial management, money handling, and consumer skills.
4. Assistance with medical supplies and prescription, clothing, seasonal needs and holiday shopping.
5. Other help with recreation and community orientation.

LIFE EDUCATION
As needed, funding provided by contract and donations.

1. In - home training in ADL by attendants and other staff.
2. Learning Center offers a range of education from coin and color identification to support for higher education.
3. Crafts classes for recreation and therapy.
4. Sexuality, peer counseling, marriage and parenting, family relations.
5. Financial mangement assistance and training: money handling, checkbood balancing, and bill paying.
6. Assistance and training in applying for and maintaing benefits: Food Stamps, SSI, rent rebates, taxes, etc.
7. Attendant - management skills: time management, assertiveness, and personal relations.

As the severly disabled people of Atlantis are liberated from nursing homes and institutions, their many needs for training and education are addressed at the Learning Center. "Special education" classes or no schooling at all left many unequipped for functioning in the community. Some people with multiple disabilities need daily reinforcement and behavior modification. People interested in entering adult education courses need support and tutoring. Many want daily socialization and recreation activities. To these ends, the Learning Center has developed the following programs:

1. Basic Education: Teaching available ranges from coin and color identification to GED preparation, along with support for people involved in college classes or other continuing education.

2. Life Education: Includes skills training on issues pertaining to living with disabilities: attendant management, advocacy and assertiveness, per "rap" groups, and others. Classes on cooking, home and money management, gardening, computers and current events are popular.

3. Crafts and Recreation: The learning Center has well - equipped crafts classes that are designed both for fun and for occupational therapy needs. Outings for concerts, movies, camping and other special events are frequent.

4. Life Skills Training: Staff design and operate programs to teach and reinforce activities of daily living, ways of coping with the stresses of living with both a physical / cognitive disability and an emotional disability.















NFT Project = (Nursing Facility Transition)

The NFT Project is a federally funded program through Medicaid, designed to broaden the long-term care choices of people with disabilities. The NFT Project provides outreach and guidance to nursing home residents who are interested in living in the community. Eligibility criteria has tree main components. The individual must; have a disability, currently living in a long-term care and skilled nursing facility, receive Medicaid, and will remain Medicaid eligible.

The Atlantis Community Incorporated NFT program is designed to assist each individual in the transition process with the understanding that the resident, will fully participate in the transition process, including housing search, benefit arrangements, and work closely with the facilities social worker. Each person will have choices to make including who they want for home health care and or personal care. The NFT project and the individual will insure that the necessary supports are in place after the individual has moved. In the philosophy of true independent living, Atlantis Community Inc., strongly encourages each transitioned individual to recruit and educate other interested parties who wish to leave their current living situations.


Apartment Modifications

Atlantis Community Incorporated and the city of Denver are working together to create greater independence for people with disabilities. The Multifamily Access Modification program was created for people who are not eligible for assistance from other modification program, i.e., HCBS. Other eligibility requirements include: income of less than $20,000.00 a year; applicant must be a resident of the City and County of Denver; and applicant must live in a multifamily unit (apartment, duplex, triplex, etc.). Home modifications include ramps, electrical changes (light fixtures), widened doorways, lowered cabinets, roll-in showers, and other modification to improve independence.


Atlantis Durable Medical Equipment (DME)

In 1995 Atlantis created a repair shop to provide wheelchair repair and sales for motorized as well as manual chairs. In addition to wheelchairs, other equipment, such as shower benches, walkers, seating, and positioning devices are available. Atlantis DME provides the most timely and highest quality service for our customers. Atlantis is reimbursed by Medicaid, Medicare, and private insurance for its services and sales.
